Care to Share – Patient Referral Program

New Patient referrals are the highest form of flattering we can receive as a practice so we have create a special program called, “Care to Share” that offers benefits to both the “referee” and the “referrer.”

If you are an existing patient and would like to refer a new patient, that individual will receive a special “Care to Share” Practice Introduction Fee of $45 for their first new patient comprehensive exam. That includes a full series of radiographs, digital photographs, and a one-on-one treatment consultation with Dr. Makrauer.

And you will receive not only a thank you from your friend, but one from us as well, such as free movie passes, gift certificates to local businesses, etc. (our thank you changes several times a year).

To be sure you receive recognition for your referral and the person you are referring receives the special “Care to Share” Practice Introduction Fee of $45 for their first comprehensive exam.

Please download and print out the Care To Share form, complete the required information and have the new patient bring this form to their first appointment.
